Living a Healthy Life with Chronic Conditions is a free, evidence-based workshop designed to help adults manage ongoing health conditions and improve quality of life. Through group discussion and practical activities, participants learn skills to better handle daily health challenges and feel more confident in their self-care. This program was developed by Stanford University’s Patient Education Research Center and is used nationwide. It is evidence-based and proven to help participants improve self-confidence and health management skills.
This workshop is for adults living with chronic conditions such as diabetes, heart disease, arthritis, chronic pain, asthma, lung disease, or other long-term health conditions.
What the Class Offers:
• Learning to problem-solve and set realistic health-related goals
• Understanding which foods support better health
• Starting or maintaining a safe exercise routine
• Improving communication with family, friends, and healthcare providers
• Creating a personal self-management plan you can use every day
Program Details:
- Six weekly sessions
- Each session is 2.5 hours
- Small group, discussion-based format
- Free participant workbook included
- Attendance at all sessions is encouraged
